Metabolic Disorders in Diabetic Patients
Metabolic disorders are common among people with diabetes, as diabetes itself is a metabolic disease that affects how the body uses and processes energy. Here’s an overview of the main metabolic disorders that can occur in diabetic patients:
֍ 1. Dyslipidemia (Abnormal Blood Lipids)
• Description: Common in both type 1 and type 2 diabetes. The body has abnormal levels of fats (lipids) in the blood.
• Typical pattern:
o High triglycerides
o Low HDL (“good” cholesterol)
o High LDL (“bad” cholesterol)
• Effects: Increases the risk of atherosclerosis, heart attack, and stroke.
֍ 2. Metabolic Syndrome
Description: A cluster of conditions that occur together, increasing risk for heart disease and diabetes complications.
• Criteria (any 3 of these):
o Abdominal obesity
o High blood pressure
o High fasting glucose
o High triglycerides
o Low HDL cholesterol
• Result: Increased risk of cardiovascular disease and insulin resistance.
֍ 3. Ketoacidosis (Diabetic Ketoacidosis – DKA)
• Mostly in: Type 1 diabetes.
• Cause: Lack of insulin → fat breakdown → buildup of ketones → blood becomes acidic.
• Symptoms: Nausea, vomiting, dehydration, fruity breath, confusion.
• Emergency: Requires immediate medical treatment.
֍ 4. Hyperosmolar Hyperglycemic State (HHS)
• Mostly in: Type 2 diabetes.
• Cause: Extremely high blood sugar without ketosis.
• Symptoms: Severe dehydration, confusion, weakness, coma.
• Emergency: Life-threatening metabolic imbalance.
֍ 5. Lactic Acidosis
• Cause: Accumulation of lactic acid in the blood, often due to poor oxygen use in cells.
• Linked to:
o Metformin (especially in kidney/liver dysfunction)
o Poor tissue perfusion
• Result: Low blood pH, weakness, rapid breathing, confusion.
֍ 6. Hypoglycemia (Low Blood Sugar)
• Cause: Excess insulin, missed meals, or over-exercise.
• Symptoms: Sweating, tremor, dizziness, confusion, loss of consciousness.
• Emergency: Must be corrected quickly with glucose intake.
֍ 7. Obesity and Insulin Resistance
• Effect: Leads to impaired glucose metabolism and worsens type 2 diabetes.
• Mechanism: Excess fat (especially abdominal) reduces insulin sensitivity.
֍ 8. Electrolyte Imbalance
• Common imbalances: Sodium, potassium, and magnesium due to dehydration or kidney dysfunction in diabetes.
• Results: Weakness, irregular heartbeat, and metabolic instability.
