WHAT IS AN AFFIDAVIT OF LOSS?
An Affidavit of Loss is a sworn statement used to formally declare that a document, identification card, certificate, receipt, or other important item has been lost.
It usually contains the following information:
1.) A clear description of the lost item or document
2.) The circumstances surrounding the loss
3.) The date and place where the item was last seen, if known
4.) The efforts made to locate or recover it
5.) A statement that the loss was not intentional
An Affidavit of Loss may be required when applying for the replacement of government issued identification cards, driver’s licenses, ATM cards, passbooks, school records, certificates, official receipts, and other important documents.
Since it is a sworn statement, all information stated in the affidavit must be true, complete, and accurate.
Providing false information in an affidavit may result in legal consequences.
