1. Pass your ๐๐๐๐๐๐๐๐ ๐๐๐๐๐๐๐๐๐ at our official email address or hand-in your printed documents at the DA-AMAD VI office:

2. Before clicking the send button, make sure you ๐๐จ๐ฆ๐ฉ๐ฅ๐๐ญ๐ all the necessary documents for the application.

Application letter addressed to the Regional Executive Director Dennis R. Arpia;

Duly accomplished Application Form;

Business Model Canvas (BMC) with Activity & Financial Plan as prescribed by the program;
โ
Clear scanned copy of at least one (1) Valid ID (indicating date of birth);
โ
Proof of Residency from any of the following:
- Barangay Certificate
- Valid ID indicating home address
โ
Endorsement letter from the Provincial / Municipal / City Agriculture or Veterinary Office
โ
Latest 2x2 ID Photo with White Background
โ
Notarized Authority to Use of Land (for release of fund)

Needed additional documents per category:
- Processing - Duly accomplished Form 2 - Suppliers Information
- Digital Agriculture - Blueprint, Autocad, among others
