Respite- Allows family members and other caregivers time for themselves, while their family members enjoys overnight or weekend stay with peers at the Farm. Supported Employment- Offers individuals an opportunity to gain competitive employment either at the Farm or at business in local communities. Day Habilitation- An intense day program for individuals who have graduated from high school and are

seeking a meaningful adult program that will assist the person in preparing for more independence in their daily living skills. Opportunities are given for community involvement and volunteer work. Community Habilitation- Assists individuals in learning and maintaining the skills necessary to remain as independent as possible in their own homes as well as supporting them out in the community. Family Support Services- Provides opportunities for parent education and individuals to participate in cultural events and weekend trips. Summer Program- Provides a 6 week program for school-aged children. This program provides stimulus for growth and responsibility in the setting of a rural farm.

No power means no water, no bathrooms, no heat, no phones, no technology.
No Day Hab, Employment or Respite services.

Employment Coordinator Position Available

Full Time: Monday through Friday, typically 8:00 am to 4:00 pm
Starting Pay: $16+/hour, based on experience at the time of hire

Position Summary: Provide direct and indirect employment services to persons enrolled in Supported Employment (SEMP), Pathway to Employment, and Community Based Prevocational Services and other employment services offered through Heritage Farm, Inc. Agency vehicle provided.

Main Position Responsibilities:
• Provide direct and indirect services and supports to individuals

• Assist service recipients with developing job readiness skills such as time and attendance, appropriate dress, communication skills, accepting supervision and travel skills.
• Provide skill training and assistance to individuals receiving employment services by learning specific work duties and performance standards for jobs service recipients.
• Facilitate the development of positive relationships between individuals and their employers and co-workers and communicate with supervisors on how to best work with newly placed individuals
• Provide timely reports on individuals' progress, including daily documentation, monthly and quarterly.

Qualifications:
• High School Diploma or GED required, Associates Degree or higher preferred
• 2-3 years related experience working with people with Developmental Disabilities
• Effective communication and organizational skills required
• Flexibility with assignments and schedule required
• Valid driver's license; proof of current auto insurance; acceptable driving record required

RE: COVID Vaccine Booster

Participants, Family, and Friends,
This message is for those of you that have had one (1) dose of Johnson and Johnson COVID Vaccine, or two (2) doses of Moderna or Pfizer Vaccine.
The effectiveness of these against COVID infection has decreased with time. Although that decrease amount is dependent on the origin of the study, they all agree that they decrease in preventing COVID Infections.

One way to get some of that protection back is to get a third injection – a booster shot. It is preferable to get that third shot with the same vaccine you originally received, i.e., If you received the Johnson and Johnson Vaccine, you should get a Johnson and Johnson Booster. Those who received Moderna should get a Moderna Booster, and those who received Pfizer should get a Pfizer Booster.
You are eligible for the Moderna and Pfizer booster shot six months after your second vaccine and two months after the Johnson and Johnson single vaccine.
Booster injections are available at all major pharmacies, Walmart’s, Walgreen’s, Kinney’s, and Rite Aid’s. There are small privately owned pharmacies that may also have booster shots available, but you will have check with them.
Almost all pharmacies that deliver the Booster injections ask that you schedule an appointment with them ahead of time. If you do not have a computer to schedule an appointment, you should contact the pharmacy by phone or in person.

AMENDED MASK REQUIREMENT
Pursuant to 10 NYCRR 2.61, The Centers for Disease Control
and Prevention (CDC) has identified a national trend of increasing
circulation of the Delta COVID-19 variant, which is approximately
twice as transmissible as the PREVIOUS strain. Since early July,
cases have risen 10-fold, and 95 percent of sequenced recent
positives in New York State were the Delta variant.
COVID-19 spreads through respiratory droplets, and several
studies have shown that appropriate face coverings/masks
reduce the spray of droplets when worn correctly, fully covering
the nose and mouth.
In keeping with the best practice policies of the CDC, DOH, and
Heritage Farm, the wearing of masks will be required while on
Heritage Farm Properties. When outdoors discretion should be
used regarding distancing and familiarity with persons around
you. This policy will be required of all participants, employees,
contractors, vendors, interns, families, and guardians. Visitors
who have essential business will also be required to wear a mask
throughout the visit. If possible, all contacts with Heritage Farm
should be done remotely.
This policy is not intended to change any other policy regarding
mask wearing, i.e. transportation and distancing. Bandannas and
Gators are not allowed. All employees must wear a mask that is
of surgical type or better. Employee/Participant/other person’s
masks that are ill fitting, torn, or dirty will be replaced with a
surgical mask provided by Heritage Farm.
If you work in an area where you are the only person in the room,
you may remove your mask; however, you must place your mask
back on if a person was to enter your office space.

Dr. Theodore Kastner, Commissioner of OPWDD, has issued these new COVID-19 precautions’ Guidance, effective immediately.

For Unvaccinated Employees, Participants, Family Members, Volunteers, etc. masks and social distancing will be required at all times for the following activities:

-All forms of transportation including to and from program services, community outings, and travel within program sites.

-For travel that includes anyone not vaccinated, capacity of the vehicle will remain at 50%.

-Continue to wear masks and maintain social distance when delivering or receiving certified services.

-Unvaccinated visitors must continue to wear masks and maintain social distance for the duration of the visit.

-All visitors must be screened in at the Farm House. If no one is there to screen you in, dial the number provided at the desk.

-Unvaccinated Participants and staff can modify their status by getting one of the authorized COVID Vaccines.

Vaccinated Employees, Participants, Family Members, Volunteers, etc.
-All forms of transportation including to and from program services; community outings; and travel within program sites must wear masks.

-If everyone in the vehicle is vaccinated, the vehicle can accommodate 100% capacity.

-Fully vaccinated Participants and staff are no longer required to wear masks or maintain social distance in OPWDD certified and operated programs.

Staff, Volunteers, Participants, Family Members, etc. will be allowed to follow the new Vaccinated Guidance as soon as they present a copy of their Vaccination Card.

-Please do not send the original card; send in a photocopy, or Fax to (315) 893-7475. Because of HIPAA laws, we cannot receive copies of medical information via mobile phones.

• Respite, per diem; shifts available weekdays, late afternoon/evening and also from Friday 3 pm - Sunday, 6 pm. Starting pay is $14.45+ per hour, depending on experience. Overnight shifts available starting at $17.45/hour.

• Day Habilitation, part-time available, Monday thru Friday 9-3, Starting pay is $12.90+ depending on experience.
