Field notes: What does breastfeeding look like when the baby goes to the NICU after birth? This is a photo of baby T that I saw after she was whisked away post birth. When I walk into a new mom’s room when her baby is in the NICU, it instantly takes me back to this moment. Everything is different to what you imagined it to be and mom can feel a little disconnected and disoriented but mom can also feel very much like she she desperately wants to do something for her baby. Breastfeeding at this moment looks like pumping every 2-3 hours followed by hand expression. Those first couple of times moms may see nothing come out once they use the pump, but follow it up with hand expression and moms will see a drop or more come out. And that visual changes EVERYTHING! The doubt that the breasts don’t have milk, that uncertain place in life where they don’t know what they are doing melts away a little. That single drop is a promise of things to come!

Tales from the field: A mom was pumping for her baby in the NICU and noticed that the fl**ge had a rim of her skin on it. Was she grossed out? A little. But is it normal? ABSOLUTELY! It is not dirt. These are oils and secretions from your Montgomery glands. Just like our noses lead us to the kitchen, these oils tell babies noses where the food is. Don't wash it off! Babies need the smells to guide them to lunch. Nom Nom!

Tales from the field: A lot of new moms worry they are not making enough milk in the first couple of days post delivery. The #1 thing that blows their mind is when asked, “When do you think the body starts making milk for the baby?” It is usually an “I don’t know” The answer is at 4 months of pregnancy!!! The milk is there. It needs a little time to trickle down. Drip, drip by dripetty drip!
